By submitting this survey, I am committing to the following requirements of the FanDuel 404 Crew:

-I am at least 21 years old
-I am not on FanDuel's self-exclusion list of bettors
-I will capture and share my experience on social media for every game and event that I attend with the FanDuel 404 Crew
-I will promote a minimum of 10 Hawks games, theme nights, or tentpole events on social media throughout the season (social graphics to be provided by Hawks)
-I will make myself available to attend at least 20 Hawks games during the 25-26 season
-I will wear Hawks gear for all games I attend with the FanDuel 404 Crew
-I will arrive to State Farm Arena at least 1 hour prior to tipoff for all games I am granted a ticket in the FanDuel 404 Crew
-I will arrive to my ticketed location at least 30 minutes prior to tipoff for all games that I am granted a ticket with the FanDuel 404 Crew
-I will stand and cheer for the Hawks for all games I attend with the FanDuel 404 Crew
-I will stay at State Farm Arena for the entirety of each game I attend with the FanDuel 404 Crew
